Medicare (EPC)

The Chronic Disease Management (formerly Enhanced Primary Care or EPC) — GP services on the Medicare Benefits Schedule (MBS) enable GPs to plan and coordinate the health care of patients with chronic or terminal medical conditions, including patients with these conditions who require multidisciplinary, team-based care from a GP and at least two other health or care providers.

Patients who have a chronic medical condition and complex care needs, and are being managed by their GP under a GP Management Plan and Team Care Arrangements are eligible for Medicare rebates for certain allied health services.

Each practitioner at The Movement Standard is registered to accept team-based care plans from GPs, providing eligible claimants with subsidised sessions; Exercise Physiologists (item 10953), Physiotherapists (item 1096), and Dieticians (item 10954). For the duration of your care, our allied health practitioners will provide your GP with reports that assist in contributing to a comprehensive team care plan. 

A Medicare rebate is available for a maximum of five services per patient, each calendar year. Eligible patients only pay the difference between a practitioner's standard fee, and the Medicare rebate.

What is a chronic medical condition?

A chronic medical condition is one that has been (or is likely to be) present for six months or longer. Some conditions include, but are not limited to: asthma, cancer, cardiovascular disease, diabetes, musculoskeletal conditions and stroke

Chronic Disease Management items are designed for patients who require an ongoing structured care approach from a multidisciplinary team, enabling GPs to effectively plan and coordinate the care of patients with complex conditions.

Eligible Patients:

Community-based patients may be eligible if they have a chronic (or terminal) medical condition and their GP has provided the following Chronic Disease Management (CDM) services:

  • A GP Management Plan (GPMP) - (item 721) and

  • Team Care Arrangements (TCAs) - (item 723).

If you believe you may be eligible to claim this service, please speak to your GP. Upon establishing eligibility, your GP will send The Movement Standard an EPC referral, enabling you to use a Medicare rebate to subsidise the referred number of sessions.
